Patient Education
For our members who suffer from Diabetes, we offer Diabetes Management classes. Please check the Events calendar for dates of scheduled classes. Diabetes Management classes will be held in two parts on consecutive Tuesdays from 2:00 p.m. – 4:30 p.m.and will be held at our Victorville High Desert Primary Care facility located at 12550 Hesperia Rd., Suite 100 Victorville, CA 92395.
To participate in this class, the patient will need a referral from their Primary Care Physician. When the referral is submitted, a Member Services representative will contact you to schedule your attendance in the Diabetes class.
For Diabetes Management (DM) Education Classes we ask that patients arrive 15 minutes before class time to sign in. For additional information please contact Member Services at (760) 956-4126 or MemberServices@HDPCMG.com.
To view the dates, times, and locations for the upcoming classes, please visit the Events page of our web site. High Desert Primary Care reserves the right to change this schedule at any time. In the event that a class is changed or cancelled, our Member Service department will contact all patients who have been scheduled to attend that class to make new arrangements.
